Request a Meeting with the Mayor/Staff

  1. Mayor Hayward receives numerous requests for meetings. To ensure request are tracked, recorded and processed in a timely manner, Mayor Hayward’s office requires that all meeting requests be submitted in writing. Meeting request should be sent a minimum of 3 weeks in advance.

  2. Please do not use this form to request the Mayor attend or speak at your event. For such requests, use the following form:

  3. e.g. mornings, afternoons, specific date

  4. Please upload an agenda or outline for the meeting.

  5. Requestor Information

  6. Once your completed request has been submitted and reviewed, you will receive a response from Mayor Hayward’s office. The completion and submittal of your request does not guarantee Mayor Hayward’s attendance or acceptance of request. If Mayor Hayward is unavailable, an individual from his staff may be available to represent the Office of the Mayor on his behalf.
